Completing an Intake Is Not Approval
Completing a medical assessment on [pepti] does not guarantee that you will be approved for treatment or receive a prescription. The assessment is a screening tool that gathers the information a licensed provider needs to make a clinical decision.
Payment Is Not Approval
Submitting payment does not guarantee a prescription or treatment. Payment authorizes the provider review and program services. If a licensed provider determines that treatment is not appropriate for you, your medication will not be prescribed and you will not be charged for the medication.
Eligibility Is Determined by a Provider
Whether you qualify for any treatment is decided solely by an independent licensed provider, based on your medical history, current medications, contraindications, applicable clinical criteria, and the law of the state where you are located. Not all patients qualify.
Continued Treatment Is Not Guaranteed
Initial approval does not guarantee ongoing or refilled treatment. A provider may modify or discontinue treatment at any time based on your response, new health information, lab results, or clinical judgment.
